The vision of Central Home Health is to promote independence, healing and comfort through quality and compassionate care to homebound patients. Central Home Health medical professionals will engage only in activities recognized to be approved medical practices. All Central Home Health employees will strive to rehabilitate each patient to his/her maximum ability and teach the patient and/or family members, necessary care for the comfort and well being of the patient as ordered by the patient’s physician.


Our Services

Our nurses are skilled in training and/or education of you and/or your family member – including disease management programs, such as diabetic teaching, COPD and CHF. We also have Psychiatric nurses readily available, on staff for management and treatment of psychiatric care.
Some of the Physical Therapy services include: gait training, creating of a home safety program for ADLs, home exercise program and maintenance program for safety strengthening.
Occupational Therapy services include environmental assessment for ADL safety, low vision therapy, safe use of DME, and teaching and training of the home health aide.
Speech Therapy services include cognitive linguistic disorder, dysphagia treatment and caregiver training.
A Medical Social Worker will help to provide support needed, to you and your family, to help to cope with injuries or illness – chronic, acute or terminal.
The Home Health Aide will help to monitor the patient’s condition by observing your physical and mental condition, your input and output, and your exercise and will report to the nurse any changes, etc.
